I also have exactly the same problem on my 2004 Sienna Limited AWD. Except this is the second set of wheels it has happen too. Toyota replaced the first set at 32,000 miles. The same time they replace my run flat tires which were recalled. When I pointed out the same exact problem again they refused to address it, since it was past the original warranty. Obviously it's a factory defect and they refuse to address it.
