Does anyone experience Slap Piston with their Toyota Sienna. My van has 250,000 and I have had had this for about a year. Is it worth fixing?

My 2009 Toyota Sienna passenger side headlight went out, I replaced bulb-didn't come on, looked at fuses-nothing blown. What else can I do fix?
